Referrers have come to recognize that we believe in prompt and courteous communication. All referrers receive a personal letter, and we often discuss cases by phone. Accessibility is also part of a properly designed referral process, and at that Middleton site we are able to see your patient within the same week, sometimes by the next day.

We are providers for the following insurance carriers:

• All private insurance, including WEA, WPS, Humana
• The Alliance Group
• Physicians Plus Insurance Corporation
• All point of service (POS) plans, including those of Unity and Dean
• Medicaid
• Medicare

No referral is needed for Physicians Plus patients. Please note that we are not providers for non-POS plans of Unity, Dean or Group Health HMO’s.

• Overuse syndromes
• Treadmill stress tests
• Arthritis

• Clearance to exercise
• Heat Illness
• Exercise-induced asthma
• Exercise in the presence of chronic disease
• Metabolic conditions arising from exercise, such as exercise-induced hyponatremia
